Volunteer attorneys trained in the process will be available to provide pro bono assistance to you with the preparation of the name-change petition and the filing.  The Monroe County Court of Common Pleas has agreed to waive the publication requirements for individuals who participate in the clinic. Filing fees may apply.

IMPORTANT: Once this form is submitted and reviewed for eligibility, you will receive an email with an appointment time as well as what your next steps will be. You must be sure to have the following documents ready for your appointment or your volunteer attorney will not be able to submit the name change petition for you:

  1. Valid, Current Photo ID
  2. Social Security Card
  3. Birth Certificate
  4. Fingerprint Card - MUST be the original.

The Monroe County Prothonotary Office and the Monroe County Court of Common Pleas have agreed to waive the filing fees for those meet specific financial requirements as determined by the Monroe County Bar Association program guidelines.
